Path Case Management I

Primary Duties and Responsibilities Full-time Will identify, locate, and engage homeless individuals which meet the homelessness definition and link them to needed to services within local mainstream community resources.
Under supervision of the Licensed Program Supervisor, the Outreach Case Management Specialist will identify homeless individuals within the local three county area and engage them for services with the ultimate goal for them to agree and obtain permanent housing.
Upon identification of needed resources, the Specialist will provide community based referrals which may include linking to needed services such as housing, medical, legal, and or other sources.
Will record individual case needs information, update case files, and provide case status information into the HMIS system in order to provide a complete record of enrollment services so as to meet requirements of this grant.
Will assure that individuals are linked into interim services if substance abuse or mental health services are unavailable at that time.
Collects, maintains, and organizes data related to specific quarterly and annual reporting requirements of the PATH grant and will train Outpatient sites so that the homeless individuals can obtain needed services.
Facilitates persons served movement through the community service delivery system, tracks and documents persons served progress, and maintains a written record of said activities.

52 and transport TCHS person served in any agency vehicle.
Performs other reasonable and related duties as assigned.
Minimum Training and ExperienceRequires a high school diploma or GED.
Two (2) years of full-time experience in a mental health or addictions treatment setting or experience with homeless individuals is preferred.
A Florida Certification Board certified recovery specialist may substitute for required years of experience.
Requires knowledge of area wide community based resources in the Polk County area, addictions treatment, and/or mental health treatment programs and supportive services Recommended Skills Case Management Community Mental Health Services Service Delivery Substance Abuse Mental Health Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
